Credit Application

Download the Credit Application. Completed applications can be submitted to FVC Credit Manager, Krista McClain at kmcclain@fvcoop.net, faxed to 308-882-3242 or mailed to FVC at PO Box 578, Imperial, NE 69033. All four pages of the Credit Application must be completed in their entirety and signed for application to be processed. Page five for Cardtrol Fuel Cards is optional. Incomplete Credit Application submissions will not be processed.

Please note, if you have a security freeze on your TransUnion Credit Report, you will need to release it before the Credit Application can be processed. If you have questions, please contact the FVC Credit Manager.
